The information below will guide you as you create your School Counseling Program Portfolio. Please place the documents behind tabs in a three-ring binder as indicated with the submission number clearly marked on each document.
The application deadline is June 1st of each year. Portfolios should be mailed to Amanda Culhan,
Academic & Career Specialist,
Indiana Department of Education, 151 West Ohio Street, Indianapolis, IN 46204.
A panel of trained practitioners will review submitted portfolios. Award recipients will be notified by email prior to June 30th of each year with a formal presentation made at the Indiana School Counselor Association Fall Conference.
By submitting your portfolio, you are giving the Indiana Department of Education permission to share and reproduced your materials with appropriate citation given to your school. All documents must be from the current academic year or last year. Documentation more than a year old will not be accepted.
